Complaints
You may use the forms below to submit complaints to the Town Administrator, Human Resources Director, or Select Board Chair.
Your complaint may be relevant to a State Agency. In many cases you may file a complaint directly with the Agency if you'd prefer. Visit mass.gov for more information. Common Agencies are: The Office of the Inspector General Fraud, Waste, and Abuse Hotline, File a Complaint with the State Ethics Commission, and the webpages for the Open Meeting Law and Office of the Attorney General.

What to expect after filing your complaint:
- Your complaint will be acknowledged in writing, including an estimated timeline for resolution.
- If applicable, your complaint will be thoroughly investigated, which may involve additional consultation with outside parties. If an investigation is not conducted, you will be notified in writing with an explanation.
- You will receive a written response outlining the outcome of your complaint, any necessary next steps, and the closure of the complaint process.

- Harassment Complaint Form - Regarding a Town Employee: Use for harassment complaints against someone employed by the Town of Carlisle.
- Harassment Complaint Form - Regarding an Elected or Appointed Official: Use for harassment complaints against someone volunteering for the Town of Carlisle, including Board/Committee members.
