Birth, Marriage, and Death Certificates
Certificates are available for a fee of $15.00 for each certificate. Payment can be made by cash, check, or online. Payment is due at the time you pick up the certificate or received before it will be mailed to you.
Please contact the Town Clerk's Office at townclerk@carlislema.gov or 978-369-6155 before purchasing online or to request your certificate and arrange a pick up date and time. Contacting the Office ensures your certificate is in the town's records before you make payment. You may also come to the office in person to request a certificate, but you will need to return to pick it up or have it mailed to you.
All Certificates are originals with unique identification numbers that have the seal and signature of the Town Clerk.
Birth Certificates are recorded and kept in the municipality of residence of the parent(s). Marriage Certificates are recorded and kept in the municipality where the Intention of Marriage was filed (not necessarily where the wedding took place or the parties maintained residence). Death Certificates are kept in the municipality of residence at time of death (not the place of death).
